CK MEDICAL STAFFING

REFERRAL PROGRAM — TERMS & CONDITIONS

Program Launch Date: October 13, 2026 Terms Last Updated: October 13, 2026 Company: Caring for Kansas Inc. DBA

CK Medical Staffing Standard Referral Bonus: $300 Gross

PROGRAM OVERVIEW

The CK Medical Staffing Referral Program rewards eligible individuals who refer qualified healthcare professionals who subsequently begin and successfully work qualifying assignments with CK Medical Staffing (“CK”). The program applies to eligible healthcare professionals across all healthcare disciplines and specialties staffed by CK, including nursing, allied health, imaging, therapy, respiratory care, and other qualifying healthcare professions. The program is open to eligible current employees, former employees, healthcare professionals, and members of the general public, subject to these Terms & Conditions. The program is prospective. Referrals made before its official Program Launch Date are not eligible for retroactive bonuses. Participation is subject to the requirements below.

Program Launch Date and Subsequent Revisions

The Program Launch Date is the original date on which the CK Medical Staffing Referral Program first became effective. This date remains fixed and does not change when these Terms & Conditions are amended or updated. The Terms Last Updated date identifies the most recent revision to these Terms & Conditions. References throughout this policy to the program’s effective date or launch refer to the original Program Launch Date unless a provision expressly identifies a different date. Revisions do not restart the referral validity period, change an existing Controlling Application, or establish eligibility for referrals made before the Program Launch Date. Revisions apply prospectively in accordance with Section 19.

1. REFERRAL BONUS

The standard referral bonus is $300 gross for each eligible referral that satisfies all program requirements. Only one referral bonus may be earned for a referred healthcare professional’s initial qualifying placement with CK. Assignment extensions, renewals, and subsequent contracts do not generate additional referral bonuses. There is no predetermined limit on the number of different eligible healthcare professionals an individual may refer. CK may offer different bonus amounts through authorized written promotions, subject to Section 18.

2. QUALIFYING ASSIGNMENTS

The CK Medical Staffing Referral Program applies to eligible healthcare professionals placed in qualifying travel or local contract assignments through CK Medical Staffing. Qualifying assignments may include, but are not limited to:

Registered Nurses (RNs).

Licensed Practical Nurses (LPNs).

Respiratory Therapists.

Physical Therapists.

Occupational Therapists.

Speech-Language Pathologists.

Radiologic Technologists and other eligible imaging professionals.

Other healthcare professionals recruited and placed by CK Medical Staffing. Eligibility is based on the assignment type and the healthcare professional’s qualifications, rather than being limited to a particular clinical specialty or profession. PRN, per diem, and individual shift assignments are excluded unless expressly included in a published promotion. CK may modify qualifying assignment categories through updated terms or authorized written promotions. The applicable bonus amount and assignment eligibility requirements are determined by the terms in effect when the applicant submits their Controlling Application.

3. REFERRED APPLICANT ELIGIBILITY

To qualify, the referred healthcare professional must:

1. Meet the licensing, certification, credentialing, and employment requirements for the applicable assignment.

2. Never have previously completed onboarding with CK Medical Staffing or Caring for Kansas Inc. – Medical Staffing.

3. Not already be actively engaged in CK’s documented recruiting or hiring process when the Controlling Application is submitted.

4. Be the subject of a genuine referral made on or after the Program Launch Date.

5. Identify an eligible referrer in the required application fields.

6. Satisfy all other program requirements.

Completed Onboarding

Completed onboarding means the individual completed CK’s required pre-employment paperwork and credentialing requirements and was documented by CK as cleared to begin an assignment. Beginning an application, submitting some documents, or beginning but not completing onboarding does not constitute completed onboarding. Anyone who previously completed onboarding is ineligible as a referred applicant, regardless of how long ago onboarding occurred or whether they ultimately worked an assignment. Individuals previously employed by CK are considered to have completed onboarding.

Historical Applicants

Individuals who applied to CK before the Program Launch Date but never completed onboarding may qualify through a separate, genuine new referral established after launch. A historical application, inactive candidate record, imported lead, or previous recruiting conversation does not automatically disqualify an individual. However, an applicant who is already actively engaged in CK’s documented recruiting or hiring process when the Controlling Application is submitted is ineligible. A historical applicant’s new referral must meet the distinct post-launch referral requirements in Sections 5 and 7.

Active Recruiting Status

An applicant is considered actively engaged in recruiting or hiring when CK’s records establish an ongoing, documented process for a position or assignment, including active candidate screening, facility submission, interviewing, credentialing, or onboarding. A historical contact, previous recruiting conversation, inactive candidate record, or general recruiting communication does not, by itself, establish active recruiting status. Eligibility will be determined using the documented circumstances existing when the Controlling Application was submitted.

4. REQUIRED REFERRAL INFORMATION AND DESIGNATED REFERRER

The referred applicant must identify their referrer when submitting an application through either:

A specific Job Card application; or

CK’s general website application. The required referral fields are: “Referred By (Name)” “Referrer Contact No.” The applicant must enter the referrer’s full name and correct phone number when submitting the application. Both fields must be completed on the Controlling Application for referral bonus eligibility. They are not required for an individual to submit an employment application without participating in the referral program. The referral bonus will be paid only to the eligible individual originally identified in the “Referred By (Name)” field on the Controlling Application. The phone number must accurately identify and allow CK to contact that same individual, subject to the correction procedures in Section 5. Only one referrer may be associated with an applicant. A referral cannot be established for bonus eligibility solely through a conversation, text message, email, social media message, or other communication outside the designated application fields. By entering a referrer’s information, the applicant represents that they have permission to provide that information to CK for referral verification and administration. CK may use the information to verify the referral, contact the designated referrer, and administer any applicable bonus, subject to applicable privacy requirements.

5. NO RETROACTIVE REFERRALS, PRE-LAUNCH REFERRALS, AND CONTACT CORRECTIONS

A. Referrals Made Before the Program Launch Date

The CK Medical Staffing Referral Program applies only to eligible referrals genuinely made on or after its official Program Launch Date. Referrals, recommendations, introductions, or recruiting activities that occurred before the Program Launch Date do not qualify for a referral bonus under this program. CK will not issue retroactive referral bonuses for individuals referred or introduced to CK before the program launched, even if the applicant later applies, accepts an assignment, or begins employment after the Program Launch Date. This restriction applies equally to current employees, former employees, and members of the general public. A referral made before launch does not become eligible merely because the applicant subsequently identifies the original referrer on an application submitted after launch.

B. New Referrals Involving Historical Applicants

An individual who previously applied but never completed onboarding may qualify through a separate, genuine referral made on or after the Program Launch Date, provided the individual is not already actively engaged in CK’s recruiting process and satisfies all other requirements. To qualify as a new referral, the post-launch communication must represent distinct referral activity that meaningfully encourages or directs the individual toward a new employment opportunity with CK and prompts renewed interest in pursuing employment with CK. Simply repeating an earlier recommendation, reminding an individual of a previous introduction, asking an individual to reapply, or having the individual submit another application does not, by itself, establish a new qualifying referral. CK must be able to document a distinct post-launch referral, including the nature and approximate date of the communication and its connection to the applicant’s renewed interest. A new application alone does not convert an old referral into a new qualifying referral. The applicant must identify the eligible referrer on the Controlling Application. Supporting evidence of a referral does not replace this requirement.

C. No Retroactive Referral Claims

Referral ownership must be established on the Controlling Application. A referrer cannot be added, replaced, or substituted after the application has been submitted. Current employees, former employees, and other participants cannot establish eligibility by subsequently claiming they referred an applicant. CK will not award referral credit based solely on a statement that an individual previously recommended CK, discussed an employment opportunity, introduced an applicant to a recruiter, or otherwise assisted in recruitment. This applies even when the individual can demonstrate that the introduction occurred before the application. If the applicant did not identify the individual in the required referral fields on the Controlling Application, the individual is not eligible for the bonus.

D. Corrections to Original Referrer Information

CK may correct a minor clerical error in the originally identified referrer’s name or phone number when the correction clearly identifies the same individual. If CK cannot contact the designated referrer, CK may contact the applicant to verify or correct that person’s phone number. Corrections are permitted only when:

The referrer remains the individual originally identified.

CK can reasonably verify that person’s identity.

The applicant confirms the corrected information when necessary.

CK documents the correction and preserves the original application information. Corrections cannot be used to:

Add a referrer when the original name field was blank.

Supply an entirely missing required phone number.

Substitute another individual.

Establish a new referral.

Restart the referral validity period.

6. CONTROLLING APPLICATION AND REFERRAL OWNERSHIP

The Controlling Application is the earliest completed application submitted by an applicant on or after the Program Launch Date, whether through CK’s general website application or a specific Job Card application.

Definition of a Completed Application

An application is considered completed when the applicant submits it through a CK-designated application channel and CK’s system successfully receives and records the submission. An application does not need to result in an interview, facility submission, credentialing, onboarding, or placement to be considered completed. An application abandoned before submission or not successfully received by CK’s system is not a completed application. Missing referral information does not prevent an otherwise successfully submitted application from becoming the Controlling Application. The Controlling Application determines:

Referral ownership.

Whether the required referral information was provided.

The applicable program terms and bonus amount.

The beginning of the 180-day referral validity period. Applications submitted before the Program Launch Date do not, by themselves, disqualify historical applicants who never completed onboarding. If the Controlling Application identifies an eligible referrer and includes the required information, that individual remains the designated referrer. If it identifies an ineligible referrer, another individual cannot be substituted. If the required referral information is missing, the applicant cannot establish referral eligibility through a subsequent application. Later applications cannot change referral ownership, create additional bonuses, or restart the referral validity period. CK’s recorded application dates and submission timestamps will determine which application controls.

7. GENUINE REFERRAL STANDARD AND VERIFICATION

Genuine Referral Standard

A genuine referral occurs when an eligible referrer, on or after the Program Launch Date, recommends CK Medical Staffing or a qualifying employment opportunity to a healthcare professional, or otherwise meaningfully introduces or directs that individual to CK for potential employment. A referral may occur through a personal conversation, text message, email, social media communication, or other legitimate means. However, the referred applicant must still identify the referrer in the required fields on the Controlling Application. Merely knowing the applicant, having a personal relationship with the applicant, or being named on an application does not, by itself, establish that a genuine referral occurred. CK may consider the circumstances of the introduction, the timing of the referral, information provided by the applicant and referrer, and relevant records when determining whether a genuine referral occurred. A referral made before the Program Launch Date remains ineligible unless a separate, genuine referral occurs after launch and all other eligibility requirements are satisfied. For historical applicants or individuals previously introduced to CK, a qualifying new referral must satisfy the distinct post-launch activity and documentation requirements in Section 5(B). The applicant’s identification of the referrer on the Controlling Application remains mandatory, regardless of any other evidence supporting the referral.

Referral Date and Application Date

The date a genuine referral occurs may differ from the date the Controlling Application is submitted. When necessary to establish eligibility, CK may verify both dates. Identification of a referrer on an application does not, by itself, establish that the referral occurred on or after the Program Launch Date.

Referral Verification

Identifying a referrer on an application does not automatically establish eligibility or guarantee payment. CK will verify:

That a genuine referral occurred on or after the Program Launch Date.

That the applicant is eligible.

That the applicant has not previously completed onboarding.

That the applicant was not already actively engaged in CK’s recruiting process.

That the required referral information appears on the Controlling Application.

That the designated referrer is eligible.

That the qualifying assignment requirements have been satisfied. CK may rely on application, recruiting, onboarding, employment, assignment, timesheet, payroll, and other relevant records. CK may request reasonable supporting information when necessary to verify a referral’s timing or authenticity.

8. REFERRAL VALIDITY PERIOD

An eligible referral remains valid for 180 calendar days from the date the Controlling Application is submitted. The referred healthcare professional must begin a qualifying assignment within that period. The individual does not need to complete the 14-calendar-day and 72-hour requirements within the 180-day period, provided the assignment begins before the referral expires. A later application does not restart or extend the validity period.

9. REQUIREMENTS TO EARN THE REFERRAL BONUS

The referral bonus is earned only after the referred healthcare professional:

1. Accepts a qualifying travel or local healthcare contract with CK Medical Staffing.

2. Begins the qualifying assignment.

3. Completes at least 14 calendar days on that assignment.

4. Completes at least 72 hours actually worked on that assignment. Both the 14-calendar-day and 72-hour requirements must be satisfied. The assignment begins on the first day the referred healthcare professional performs paid work for the qualifying assignment, including paid orientation when it forms part of that assignment. That day counts as day one. The calendar-day requirement is satisfied at the end of day 14, provided the assignment remains in effect. Paid orientation hours actually worked count toward the 72-hour requirement. PTO, cancellation pay, guaranteed pay for unworked hours, and other payments for time not actually worked do not count. CK will use assignment records, approved timesheets, and payroll records to verify eligibility. Hours from unrelated assignments cannot be combined unless an exception is authorized under Section 10.

10. ABSENCES, FACILITY CANCELLATIONS, AND ASSIGNMENT INTERRUPTIONS

Approved absences, facility-canceled shifts, scheduling changes, and similar circumstances outside the referred healthcare professional’s reasonable control do not automatically disqualify an otherwise eligible referral. If the assignment remains active but the individual has not completed 72 hours by the end of day 14, the referral remains eligible until the required hours are completed, subject to the remaining program requirements. If a facility ends the assignment before both requirements are satisfied for reasons outside the individual’s control, CK may:

Approve the bonus as an exception.

Allow qualifying days or hours to carry over to a replacement assignment.

Determine that the bonus has not been earned.

Exception Review Criteria

CK may consider:

The reason for the assignment cancellation or interruption.

The number of calendar days and verified hours actually worked.

The referred healthcare professional’s compliance with assignment requirements.

Whether a qualifying replacement assignment is available.

Applicable legal or contractual obligations.

CEO-Only Exception Approval Authority

All exceptions under this section require the express written approval of CK Medical Staffing’s Chief Executive Officer (CEO). The CEO has sole authority to approve, deny, or establish the conditions of an exception involving assignment-duration requirements, hours-worked requirements, facility cancellations, or assignment interruptions. No other CK employee, officer, manager, or representative may independently authorize an exception under this section. The CEO’s written approval must identify the circumstances, decision, supporting rationale, and any remaining qualification requirements. An approved exception may modify the ordinary assignment-duration or hours-worked requirements only to the extent expressly stated in the written approval. All other program eligibility requirements remain in effect, including applicant eligibility, genuine referral status, referral ownership, and required application documentation. An exception does not authorize retroactive referral credit, substitution of a different referrer, or waiver of the genuine-referral requirement. Exceptions will be evaluated consistently with these Terms & Conditions and applicable law. An exception granted in one case does not automatically establish entitlement to an exception in another case. Legally protected leave and other protected circumstances will be handled in accordance with applicable law.

11. DISQUALIFYING CIRCUMSTANCES

Subject to Section 10 and applicable law, a bonus will not be earned if, before satisfying the qualification requirements, the referred healthcare professional:

Fails to begin the assignment.

Voluntarily resigns before completing the requirements.

Abandons the assignment.

Is terminated for documented cause.

Has documented material attendance or reliability issues resulting in assignment termination.

Fails to maintain required licenses, credentials, or employment eligibility.

Provides materially false or misleading information.

Otherwise becomes unable to continue for reasons attributable to their own conduct or failure to meet assignment requirements. Once a bonus has been earned, a subsequent resignation, termination, or employment-status change does not, by itself, cancel the earned bonus. CK may review suspected fraud or material errors concerning whether the original requirements were satisfied.

12. NO SELF-REFERRALS

Self-referrals are prohibited. The designated referrer must be a separate eligible individual who genuinely referred the applicant. An applicant cannot identify themselves or arrange for another person to claim a bonus on their behalf for a referral that did not genuinely occur.

13. REFERRAL ABUSE OR MANIPULATION

CK may deny referrals reasonably determined to involve fraud, fabricated information, material misrepresentation, or attempts to circumvent program requirements. Prohibited conduct includes:

Inventing a referral relationship.

Falsely representing a pre-launch referral as a new referral.

Submitting duplicate applications to change referral ownership.

Using another person’s identity without authorization.

Misrepresenting prior onboarding, employment, or recruiting history.

Arranging for an ineligible individual to receive referral compensation indirectly. Determinations will be based on available records and relevant circumstances.

14. REFERRER ELIGIBILITY

Subject to the exclusions below, eligible referrers may include:

Current CK Medical Staffing employees and healthcare professionals.

Former CK Medical Staffing employees and healthcare professionals.

Other healthcare professionals.

Friends and family members.

Members of the general public. Referrers must be at least 18 years old when the Controlling Application is submitted. The following individuals are excluded:

CK Medical Staffing owners and officers.

CK employees whose regular responsibilities include recruiting, candidate sourcing, hiring, or administering or approving referral bonuses.

Determination of Referrer Eligibility

For purposes of determining referrer eligibility, the relevant date is the date the referred applicant submits the Controlling Application, except where applicable law or an expressly applicable written promotion requires otherwise. The referral itself must nevertheless have genuinely occurred on or after the Program Launch Date. A change in the referrer’s employment status or responsibilities after the Controlling Application is submitted does not, by itself, alter eligibility established at that time. An excluded individual cannot circumvent these restrictions by arranging for another person to claim credit for a referral made in the course of excluded duties. Employee participation is voluntary. An eligible referrer’s subsequent separation from CK does not automatically eliminate referral eligibility or an earned bonus.

15. HEALTHCARE COMPLIANCE AND BUSINESS CONFLICTS

No referral bonus will be offered or paid in exchange for directing patient referrals, influencing clinical decision-making, improperly inducing the purchase or use of healthcare services, or improperly influencing a healthcare facility’s selection of staffing vendors. Individuals who participate in or influence healthcare facility staffing, purchasing, procurement, contracting, vendor selection, or MSP/VMS decisions involving CK may be ineligible when payment would create an improper financial incentive or violate applicable law, facility policy, or contractual obligations. CK may exclude particular individuals, facilities, referral arrangements, or categories of participants when reasonably necessary for legal or contractual compliance. CK may request information necessary to evaluate a potential conflict.

16. PAYMENT TIMING AND METHOD

CK will verify eligibility and issue an earned referral bonus within 14 business days after the later of:

The date the referred healthcare professional satisfies the qualification requirements.

The date CK receives complete and accurate recipient information and tax documentation reasonably necessary to issue payment. Verification is included within this timeline and does not establish a separate, indefinite waiting period. CK will conduct eligibility reviews promptly and in accordance with applicable wage-payment laws. If missing information or a genuine eligibility discrepancy prevents payment, CK will notify the referrer within that period, explain the issue, and provide an expected resolution date. A bonus determined to be payable will be issued within 14 business days after the issue is resolved and required documentation is received, or sooner if applicable law requires. Business days are Monday through Friday, excluding federal holidays.

Compliance With Applicable Payment Deadlines

Nothing in this section postpones payment beyond a deadline imposed by applicable law. CK will not delay payment solely because an internal eligibility review remains incomplete when applicable law requires payment sooner. Any referral bonus determined to constitute wages or other compensation subject to mandatory payment deadlines will be paid in accordance with those requirements.

Current Employees

Referral bonuses will be processed through CK payroll as taxable compensation, subject to applicable withholding and reporting requirements.

Nonemployees

Eligible bonuses will generally be issued through a prepaid gift or debit card, subject to required documentation, applicable law, and administrative processing requirements. If a prepaid card cannot reasonably or lawfully be issued, CK may use another authorized payment method.

Former Employees

An otherwise earned referral bonus remains payable following separation. CK will determine the appropriate payment method and tax treatment based on the nature of the payment and applicable law. Separation from employment does not, by itself, change the tax classification of a payment.

Referral Bonus Value and Tax Treatment

The standard referral bonus is $300 gross. Any prepaid card or other noncash payment will be issued and reported in accordance with applicable tax and withholding requirements. The stated bonus amount does not guarantee a particular after-tax value or require CK to cover a recipient’s individual tax liability. CK may use another authorized payment method when necessary to comply with applicable payment or tax requirements.

17. TAX DOCUMENTATION AND REPORTING

Referral bonuses are generally taxable income. Bonuses paid to current employees will be processed under applicable payroll and employment tax rules. Nonemployee recipients must provide a completed Form W-9 or other appropriate tax documentation when requested before payment can be issued. CK will maintain payment records and issue required tax reporting forms. The use of a prepaid card does not make a referral bonus tax-free. Recipients are responsible for their individual tax obligations.

18. PROMOTIONAL REFERRAL BONUSES

CK may offer increased bonuses or expanded eligibility for specified positions, specialties, professions, locations, or promotional periods. Each promotion will identify its applicable bonus amount, eligibility requirements, and dates. Unless the published promotion expressly states otherwise:

It applies only to Controlling Applications submitted during the promotional period.

The bonus amount is established when the Controlling Application is submitted.

The promotional bonus replaces, rather than adds to, the standard bonus.

Promotions cannot be combined.

Promotional increases do not apply retroactively. All other program requirements remain in effect unless expressly modified by the published promotion.

Promotional Offer Documentation

Any promotional referral bonus must be documented in writing and identify:

The promotional bonus amount.

The effective start and end dates.

The eligible assignments, professions, specialties, locations, or applicant categories.

Any eligibility requirements that differ from the standard program. CK may publish promotional offers through its official website, social media accounts, recruiting announcements, or other authorized company communications. CK will retain a copy of the applicable promotional terms for verification and administration. Verbal statements or informal communications do not modify the program’s bonus amount or eligibility requirements unless confirmed through an authorized written promotion.

19. PROGRAM CHANGES OR DISCONTINUATION

CK may modify, suspend, or discontinue the program prospectively. An eligible referral established before a change takes effect will remain governed by the material terms and bonus amount in effect when its Controlling Application was submitted, except where changes are required by applicable law. Changes will not retroactively reduce an established referral’s bonus or add new qualification requirements unless required by law. Earned bonuses remain payable. CK will identify the effective date of revised terms and retain prior versions.

20. REFERRAL ADVERTISING, REPRESENTATIONS, AND INCENTIVE DISCLOSURE

Participants may share publicly available CK employment opportunities but may not make false or misleading statements about CK, its positions, compensation, benefits, or employment opportunities. Participants may not guarantee employment, placement, assignment availability, or assignment acceptance. Participants may not represent themselves as authorized CK recruiters, employees, agents, or representatives unless separately authorized. Use of CK’s name, logo, or other branding in independently created promotional materials requires prior authorization. Accurate ordinary personal communications identifying CK as the staffing agency being recommended are permitted.

Referral Incentive Disclosure

Participants who publicly promote CK, recommend its employment opportunities, or endorse the company in connection with this referral program must clearly disclose that they may receive compensation through the program. The disclosure must be clear, understandable, and reasonably noticeable to the intended audience. When a participant posts a referral recommendation on social media, the disclosure should appear with the recommendation rather than solely in a profile, separate webpage, or unrelated communication. An acceptable disclosure is: “I may receive a referral bonus if you apply through my referral and meet CK Medical Staffing’s program requirements.” Participants must comply with applicable advertising, endorsement, and consumer protection requirements, including applicable Federal Trade Commission guidance. CK may require correction or removal of promotional communications that fail to disclose the potential financial incentive or otherwise misrepresent the program.

21. PROGRAM ADMINISTRATION AND ELIGIBILITY REVIEWS

CK administers the program and determines eligibility based on these Terms & Conditions and relevant records. CK may investigate unusual circumstances, conflicting information, suspected errors, or potential abuse before authorizing payment, subject to Section 16. An individual who believes an eligibility decision was based on incorrect information may request a review and provide supporting documentation. CK will review the information and explain its determination. Administrative discretion does not permit CK to disregard an earned payment obligation or applicable law. If an earned bonus cannot be delivered because the recipient cannot be located or does not provide required documentation, CK will document reasonable contact attempts and handle outstanding payment obligations in accordance with applicable law, including unclaimed-property requirements. CK will protect applicant and referrer information in accordance with applicable privacy requirements. CK is not required to disclose confidential applicant employment, recruiting, or assignment information to a referrer to explain an eligibility decision. Participation does not guarantee employment, placement, assignment availability, assignment duration, or continued employment. Participation does not create an employment relationship between CK and a nonemployee referrer.

22. GOVERNING LAW, SEVERABILITY, AND NON-WAIVER

These Terms & Conditions will be interpreted in accordance with applicable law, including mandatory federal, state, and local requirements. To the extent legally permissible, Kansas law will govern their interpretation, without overriding mandatory laws applicable in another jurisdiction. Disputes will be handled in a court or other forum having jurisdiction under applicable law. Nothing in these Terms & Conditions waives rights or remedies that cannot lawfully be waived. If any provision is determined to be invalid or unenforceable, the remaining provisions will remain effective to the extent permitted by law. An exception or decision not to enforce a provision in one instance does not automatically waive that provision or require CK to grant the same exception in another instance.
